Immunization Requirements for the 2014-2015 School Year

 

 

Immunization Requirements that must be on file with school nurse before starting school are as follows:

 

Pre–K

 

If your child is 3 years of age:

  • At least three (3) doses of oral polio are required.
  • At least four (4) doses of DTaP vaccine are required.
  • One (1) dose of MMR (measles, mumps & rubella) vaccine is required.It must have been received on or after the 1st birthday.
  • Three (3) doses of Hepatitis B vaccine.
  • Two (2) doses of Hepatitis A, the first dose must have been received on or after the 1st birthday.
  • One (1) dose of Varicella (Chickenpox) Vaccine on or after the 1st birthday.Or a parent- or physician-validated history of Varicella illness (chickenpox).If your child has had Chickenpox, please ask for a form that will be necessary to have on file.
  • Three (3) doses of Hibwith a booster after 12 months of age –OR- One (1)

          dose of Hib on or after 15 months of age.

  • Four (4) doses Pneumococcal (PCV-7 or PCV-13) or Prevnar –OR- Three (3)

          doses  with one dose on or after 12 months of age –OR- two doses with both  

         doses on or after 12 months of age, or one dose on or after 24 months of    

         age.

     

    If your child is 4 years of age:

    • At least three-four (3-4) doses of oral polio are required, provided at least one dose has been received on or after the fourth birthday.
    • At least four-five (4-5) doses of DTaP vaccine are required, provided at least one dose has been received on or after the fourth birthday.
    • Two (2) doses of MMR (measles, mumps & rubella) vaccine are required.Both doses must have been received on or after the 1st birthday and 28 days apart.
    • Two (2) doses of Varicella (Chickenpox) Vaccine. Both doses must have been received on or after the 1st birthday. Or a parent- or physician-validated history of Varicella illness (chickenpox).If your child has had Chickenpox, please ask for a form that will be necessary to have on file.
    • Three (3) doses of Hepatitis B vaccine.
    • Two (2) doses of Hepatitis A, the first dose must have been received on or after the 1st birthday.
  • Three (3) doses of Hibwith a booster after 12 months of age –OR- One (1) dose of Hib on or after 15 months of age.

    Kindergarten:

Immunizations needed before entering 7th Grade:

 

  • One (1) dose Tdap booster (Students will be required to have a booster dose of Tdap only if it has been five years since their last dose of tetanus-containing vaccine. Td is acceptable is lieu of Tdap if a contraindication to pertussis exists.)
  • Two (2) doses of Varicella (Chickenpox) Vaccine after the 1st birthday.  Or a parent- or physician-validated history of Varicella illness (chickenpox).  If your child has had Chickenpox, please ask for a form that will be necessary to have on file.     
  • One (1) dose Meningococcal Vaccine

 

Immunizations for 8th-12th Grade:

Students in grades 8-12 are required to have a booster dose of Tdap if it has been ten years since their previous dose of a tetanus-containing vaccine.  Td is acceptable in lieu of Tdap if a contraindication to pertussis exists.

 

 

You may obtain these immunizations at the following:

 

  • Your family physician

     

  • Hardin County Health Unit in Kountze if uninsured, on Medicaid or Chips. Please call before going to see if your child qualifies to receive immunizations there- Phone # 246-5188. Monday & Tuesday from 7-11am & 1-4pm, Wednesday & Thursday 8-11am and 1-5pm; and Friday 8-11am. There is a $12.00 for 1st vaccine, then $2.00 for each additional vaccine; no one will be refused if unable to pay.

 

  • Beaumont Health Department on Washington Blvd. in Beaumont only if uninsured. Please call before going to see if your child qualifies to receive immunizations there- Phone # 832-4000. Monday through Friday 8-11am & 1– 4pm. You do not have to be a resident of Jefferson County to receive immunizations.There is a $10.00 charge per visit.

 

  • China-Community Clinic on Railroad Ave in China, if uninsured, or if China Community Clinic is the Primary Care Physician.Please call before going to see if your child qualifies to receive immunizations there. Phone # 752-2301.Monday - Friday from 9am-12:00 & 2-5pm.There is a $12.00 charge for each immunization. Take your immunization records with you for their files.

       

  • Minute Clinic in CVS Pharmacy 2950 Dowlen Rd in Beaumont. Accepts most insurance, some Medicaid & does not need a Physician’s order. 1-866-389-2727. Monday - Friday from 8:30am-7:30pm, Saturday 9am-5:30pm, & Sunday 10am-5:30pm. They can administer immunizations to children as young as 18 months. No co-pay for immunizations.

               

Please send the record to school, A copy will be made and the original will be returned to you.  The immunization record may also be faxed to the school.  The fax number is 409-287-3987.
